If you have an original Citroën accessory head-rest to fit to a 2cv seat (or similar type of seat frame) here is some info that might be useful.
First is the method that I have used on several seats. Next is the original German instruction sheet, followed by a Google translation of its text.

The method I use for aligning and fitting Citroën accessory head rests:
- With the seat out of the car, remove the backrest material. Then fit the seat back in the car.
- Get someone to hold the headrest in position while you’re sitting in the seat, to make sure you establish the best position (angle) to suit your normal seating position. It’s much easier to gauge this if you’re actually sitting in the car, rather than sitting on a removed seat.
- Wrap some pieces of masking tape around the tubular frame in line with the positions of the holes in the headrests mounting bar. Then, in order to get the mounting angle correct, mark the position on the masking tape at the top, (through the holes in the headrest mounting bar), and also underneath, so that you will be able to see the angle that you need to drill. When drilling, the masking tape will reduce the likelihood of the drill slipping.
- After you’ve drilled holes (centrally through the tube), smooth off any sharp edges with a file and refit the cover temporarily and feel through it for the holes, mark those positions and take the cover back off.
- To make the holes in the material, it’s best to place it over a block of wood and use an upholsterer’s punch, (6mm or 1/4″). This will give a perfectly round hole, so you’re less likely to get tearing than if you were to use scissors or a blade.
- Mount the headrest to the frame, sandwiching the backrest cover between, then after tightening the fixings, you can pull the cover down into place. (If you fit the cover all the way down, before attaching the headrest, it’s more difficult to get the washers and nuts on and tighten them).

The headrest is connected to the cross tube of the backrest tube frame, which can be felt from above
(see sketch).
- Determine and mark the centre of the backrest using the self-adhesive labels provided.
- Place the crossbar with the support rods on the backrest frame so that the marking hole lines up with the marking line on the self-adhesive label and align it so that the support rods are vertical and the mounting holes are centred above the cross tube.
- Mark the mounting holes on the backrest cover and punch through the cover.
- Cut out the backrest cover around the grain (approx. 12mm) so that no upholstery gets stuck on the drill.
- Drill through the cross tube with 6.5mm (Use the crossbar as a template).
- Remove self-adhesive label.
- Loosen the points of the backrest cover.
- Feed the U-profile with nut to the fastening holes and screw it to the crossbar through the cross tube of the backrest tube frame from above.
- Attach the cover pad.
- Place the head cushion on the support rods and snap it into place.
- Reattach the backrest cover.
The safety headrest is height adjustable in locking levels and tilt adjustable”.
